Our annuity plan builds long-term retirement security based on your covered hours of field experience. While our annuity fund is overseen by Southern Benefit Administrators, your actual individual retirement account tracking, balances, and statements are powered by Empower Retirement.

If you are married and wish to designate someone other than your spouse as your primary beneficiary, your spouse must consent, and the form must be witnessed by a Notary Public.

Because this is a legal document (and requires a Notary signature in some cases), SBA requires the physical, signed original form. Digital copies or emails cannot be accepted. Please submit the original by mail or in person:
